Voting Form Example
To Create A Voting Form Like The One Above:
- Go to Forms > New Form
- Go to Advanced Fields > Survey
- Add one Survey field per item you plan to add to the Voting Form
- Set each field’s Survey Field Type to Radio Buttons
- Add an Email field
- Check the Email field’s No Duplicates option
- Go to Standard Fields
- Add one HTML field
- Type a Disclaimer in the HTML field’s Content area. In the above example, the disclaimer is, “Vote intended for entertainment purposes only.”
- Hit Update to Save the Form
- Go to Settings > Confirmations
- Choose the confirmation message you want visitors to see when they submit their answer. In the example above, the default confirmation message was edited to say, “Thanks for voting!” To edit the confirmation message:
- Click Default Confirmation
- Edit the Message field so the message you’d like website visitors to see appears
- Hit Save Confirmation.
- Go to Settings > Notifications
- Decide whether you would like your site’s admin email address to receive a notification each time someone submits the survey. If you would like to change the email that receives a notification:
- Click Admin Notification
- Edit the Send To Email field so the email address that should receive notifications is listed
- Hit Update Notification
- Once your form is ready, you can place it on a page or in an article.
